You recently purchased a MAPP (methylacetylene-propadiene propane) cylinder to help fix a leak in the basement. While using your cylinder, you accidently drop it causing a series of flash fires which nearly kill you.

The first flash fire was caused by a faulty leak resulting from corrosion in the neck of the cylinder. The second flash fire is caused by the cylinder penetrating the ground as you drop it from your grasp. A defect in the design of the “fracture grove” in the neck of the MAPP gas cylinders is meant to fracture the tube so that the cylinder does not break and explode. However, the device does not perform its intended task, causing stress to the vulnerable portion of the cylinder. This faulty design has caused severe and sometimes fatal burn injuries to the user.
