A 46 year-old mother died of breast cancer after her primary care physician and radiologist failed to read an ultrasound correctly, which was ordered because of abnormalities in her left breast. After seeking a second opinion from another doctor, she was told that the abnormality was cancerous and underwent immediate treatment.
