The latest of these just came to light in March 2010, when the Nuclear Regulatory Commission (NRC) hit the Philadelphia VA Medical Center with $227,500 in penalties, one of the largest fines ever levied against a medical facility.

Federal investigators found that, between 2002 and 2008, the Medical Center misplaced radioactive seeds in 97 out of 116 procedures when treating patients with prostate cancer. The NRC report cited lack of management oversight and negligent safety procedures at the facility.
